Articaine with epinephrine (1:100000) 4% is a combination of an amide-type local anesthetic (articaine) and a vasoconstrictor (epinephrine). The injection of this combination for intraoral submucosal infiltration is a sterile aqueous solution containing articaine hydrochloride 4% (40 mg/ml) and epinephrine bitartrate in an epinephrine concentration of 1:200000 or 1:100000. Articaine hydrochloride is an amino-amide local anesthetic in the form of a racemic mixture. Articaine blocks the initiation and conduction of nerve impulses, presumably by increasing the electrical excitation threshold in the nerve, slowing the propagation of the nerve impulse, and decreasing the rate of action potential rise. Epinephrine, as a vasoconstrictor, is added to articaine to slow its absorption into the systemic circulation, which prolongs the maintenance of an active concentration in the tissues. The onset of anesthesia occurs 1-9 minutes after injection of the articaine + epinephrine combination. Full anesthesia lasts approximately 1 hour with infiltration anesthesia and up to 2 hours with block anesthesia. Use The articaine + epinephrine combination is indicated for local, infiltration, or block anesthesia in simple and complex dental procedures in adults and children aged 4 years and older. Articaine preparations with an epinephrine concentration of 1:100000 are used for prolonged and traumatic dental interventions requiring prolonged pain relief and active hemostasis to reduce bleeding.
