Use Diroton is used to treat essential arterial hypertension (high blood pressure). It is also used for symptomatic treatment of heart failure, for short-term treatment (within 6 weeks) of hemodynamically stable patients with acute myocardial infarction (no later than 24 hours after infarction). The drug is indicated for the treatment of kidney diseases in patients with arterial hypertension, type 2 diabetes mellitus, and early nephropathy. How to take The initial dose of Diroton is 5 mg orally, then 5 mg after 24 hours, 10 mg after 48 hours, and then 10 mg once a day. In case of low systolic blood pressure (≤120 mmHg) or within the first 3 days after myocardial infarction, a low dose (2.5 mg/day) is indicated. In case of renal insufficiency (creatinine clearance <80 ml/min), the initial dose of Diroton should be adjusted according to the patient's creatinine clearance. For patients not taking other antihypertensive drugs, the recommended initial dose is 10 mg per day. The usual maintenance dose is 20 mg/day; depending on blood pressure, the dose can be increased to 40 mg/day. Contraindications - Diroton is contraindicated in case of allergy to lisinopril or any other component of the drug. - History of angioedema (severe allergic reaction with swelling of the face, lips, tongue) associated with the use of ACE inhibitors, hereditary or idiopathic Quincke's edema. - Pregnancy and breastfeeding. - Severe renal insufficiency (creatinine clearance <30 ml/min), hepatic encephalopathy or severe liver dysfunction, hypokalemia. - Concomitant use with angiotensin II receptor antagonists (ARBs) in patients with diabetic nephropathy. - Concomitant use with neutral endopeptidase inhibitors (e.g., sacubitril-containing drugs) due to a high risk of angioedema.
